Waterford responses below - almost exactly the same as ANO, differences due to RE staff size and year round refueling group at ANO. Clint 1. What role, if any, does Quality Assurance or Quality Control play in the fuel receipt process? Specifically, do they perform any inspections, review paperwork, etc? None. QA usually does spot checks associated with new fuel receipt/procedure completion, but is not directly required for new fuel receipt. 2. What is the purpose the new fuel inspection activity? Is the new fuel receipt inspection considered an inspection for "shipping or handling damage" or it is a traditional receipt inspection? This inspection is for shipping and handling damage, as well as to look for debris, grid issues, etc. 3. Who does "inspection" (i.e. Reactor Engineering, QA/QC, Maintenance, shared resources, etc.)? Reactor Engineering and shared resources perform the inspections. 4. What specific role(s) does Rx Engineer perform? Reactor Engineering performs inspections, determines the location for new fuel, and provides oversight of the new fuel receipt process. 5. Who unloads shipping containers? Mechanical maintenance under the direction of the refueling supervisor or reactor engineering. 6. Does your plant utilize the same process for fuel receipt at the site as other procured material? No. Other procured materials go through the warehouse. 7. Does your plant require the fuel receipt personnel to be ANSI/ASME N45.2.6 qualified, and at what level? If not required to be qualified to the N45.2.6, why not? Not currently as its not required under current QA program manuals. Entergy is examining the applicability of ANSI/ASME N45.2.6 to our fuel receipt inspections and whether it would benefit the receipt process or if it is required.

It is a combination of both. We do inspect the shipping contaier's for shipping damage; plus we inspect the Fuel Assemblies and any components are inspected as well. We document both the container and the fuel Assemblies and Fuel Related Components in our FHI. 3. Who does "inspection" (i.e. Reactor Engineering, QA/QC, Maintenance, shared resources, etc.)? The inspection are performed by trained AUO to Level II, with reactor engineering Level II help when problems are identified. Also, The Reactor Engineering Supervisor and a Corporate Nuclear Fuel project lead are both qualified to Level III. 4. What specific role(s) does Rx Engineer perform? Coooridinate the shipping dates, schedule the receipt activities and support organizations such as rad con, FME personel, fire watches boiler makers and security coverage when needed. Ususally, maintains the paper work for the fuel receipt actvities. RXE, also generates the Fuel Assembly Transfer Forms used for the receipt activities. 5. Who unloads shipping containers? Most of rthe time boiler makers unload the cask to the refuel floor. Operations generally replaces the empty containers upon the truck bed after Rad Con has performed a survey on the empty bed. 6. Does your plant utilize the same process for fuel receipt at the site as other procured material? No, only fuel and fuel related components go through this process. 7. Does your plant require the fuel receipt personnel to be ANSI/ASME N45.2.6 qualified, and at what level? If not required to be qualified to the N45.2.6, why not? Mark, Here are the answers for Cooper. 1. QA reviews qualifications (experience/education background) and certifies fuel inspectors upon completion of the vendor training. They do not perform any inspections, but move sheets and inspections documents are considered quality documents. 2. Fuel inspection is not treated the same as a standard receipt inspection and is covered by Fuels and Reactor Engineering procedures. The inspections look for shipping and handling damage, as well as FME and visible manufacturing defects (scratches greater than 2 mils, issues with plenum springs, etc.). 3. Individuals at Cooper who meet QA requirements (see #1) and can qualified to inspect new fuel. 4. The RE at Cooper plans and coordinates, and implements fuel receipt and inspection. Although the SRO is the ultimate authority on the floor, the RE acts as a second supervisor and signs as SNM checker on the move sheets. Also, all of our REs are qualified as Level II fuel inspectors. 5. Maintenance unloads the containers under the supervision of the RE and an SRO. 6. No. Fuels and Reactor Engineering procedures specifically govern receipt/inspection of new fuel, control blades, and fuel channels. 7. Yes, our QA department requires us to meet those requirements. In addition, they worked in conjunction with our inspection personnel to perform the actual fuel receipt inspections. Nowadays, Clinton Reactor Engineering is responsible for review of the fuel receipt paperwork including the PQC verification. Quality Assurance is no longer in the business of supporting fuel receipt activities under Exelon Management. 2. What is the purpose the new fuel inspection activity? Is the new fuel receipt inspection considered an inspection for "shipping or handling damage" or it is a traditional receipt inspection? ------------------------ Response: The purpose of fuel receipt inspection is to inspect for shipping and handling damage and as well obvious manufacturing errors (i.e. poor engagement of expansion springs to tie rods, bent spacer tabs, etc.). 3. Who does “inspection” (i.e. Reactor Engineering, QA/QC, Maintenance, shared resources, etc.)? ------------------------ Response: The actual inspection of the fuel is done entirely by Mechanical Maintenance under the management of Exelon. Prior to Exelon, it was done by a combination of Mechanical Maintenance and Quality Assurance. 4. What specific role(s) does Rx Engineer perform? ------------------------ Response: In previous management, the assigned Reactor Engineer was actually the project manager for fuel receipt activities and coordinated all of the details of ensuring a successful receipt and inspection campaign. Reactor Engineering nowadays supports the project manager (Reactor Services) by preparing the move sheets, overseeing the logistics (contacting the vendor if needed), and when necessary supporting Mechanical Inspection with inspections. The role of the Reactor Engineer is merely now a support organization to ensure that the process is working. 5. Who unloads shipping containers? ------------------------ Response: Shipping containers are unloaded by the Mechanical Maintenance Department. 6. Does your plant utilize the same process for fuel receipt at the site as other procured material? ------------------------ Response: No, Fuel Receipt Activities are actually governed under their own procedures and processes that is connected but is entirely separate from the procecess of procuring materials. 7. Does your plant require the fuel receipt personnel to be ANSI/ASME N45.2.6 qualified, and at what level? FENOC is trying to develop a standard fuel receipt process between BWRs/PWRs. To that end, I have some questions on how fuel receipt is done at your plants: 1. Specifically, do they perform any inspections, review paperwork, etc? A1 - Neither QA nor QC play a role in fuel receipt activities or paperwork. 2. What is the purpose the new fuel inspection activity? Is the new fuel receipt inspection considered an inspection for "shipping or handling damage" or it is a traditional receipt inspection? A2 - Nuclear fuel and associated fuel components are inspected upon receipt at each nuclear site to accomplish the following; A2.1 Confirmation the fuel design and bundle serial numbers are as ordered A2.2 Confirmation that there has been no shipping or handling damage A2.3 Confirmation of certain design or fabrication details are as specified by Nuclear Fuels (NF) A2.4 Confirmation that there is no foreign material present and that overall cleanliness is acceptable A2.5 Cleanup of any minor foreign material or surface contamination A2.6 Correction of minor issues such as lock tab engagement on tie rod nuts or BWR bundle twist 3. Who does “inspection” (i.e. Reactor Engineering, QA/QC, Maintenance, shared resources, etc.)? A3 - Up until ~2000, the Reactor Engineers performed the Fuel Receipt inspections utilizing the Fuel Handling Department to move and open the containers and move the fuel. The receipt inspections were then turned over to the Fuel Handling Department, who are now called Reactor Services. Reactor Services performs the receipt inspection of the fuel or core component through use of qualified inspectors (Qualified by Reactor Engineering Annually) working to station procedures that as a minimum accomplish all requirements of the receipt procedure. 4. What specific role(s) does Rx Engineer perform? A4 - Reactor Engineering provides the interface between the corporate Nuclear Fuels Fuel Reliability Engineer, the fuel vendor and the organization (Reactor Services) directly responsible for inspecting fuel and associated core components and assures all fuel bundles and associated core components are inspected and the inspection results are reconciled to the design documents and all identified discrepancies are resolved. 5. Who unloads shipping containers? A5 - Reactor Services 6. Does your plant utilize the same process for fuel receipt at the site as other procured material? A6 - Fuel receipt does not utilize the same process. All other procured materials goes through the Stores Department and utilizes QC for safety related parts and equipment. The fuel is taken directly to the Fuel Handling Building to be unloaded and begin the receipt process. 7. Does your plant require the fuel receipt personnel to be ANSI/ASME N45.2.6 qualified, and at what level? If not required to be qualified to the N45.2.6, why not? A7 - Not currently.
