to utilize more informal benchmarking (i.e. this mailing list) to try to
improve SFP related activities and controls. Over the last few weeks I’ve
kind of been spamming this list but I feel your responses will greatly
help Ginna going forward. Thank you for all your responses to this point.

our last set of questions for a while. This time the questions are mostly
from our Director of Reactor Services and relates to fuel movement.

I am looking for information related primarily
to Non-Outage fuel movement in the SFP.
1. What is the level
of Operations involvement in fuel movement?
Response
- Operations verifies that Tech Specs are satisfied in order to perform
fuel movement
· Is
fuel movement at your site performed by Operations or by Vendor?
Response - Currently, the fuel
vendor performs all fuel moves at the site.
·
If Vendor, what group provides primary
oversight?
Response - We have a qualification
called Fueling Handling Supervisor. Personnel from any site group may be
qualified to fulfill this position. It is mainly used for supporting refueling
outage, but can also be used for non-outage fuel moves. Reactor Engineering
would supply oversight for fuel inspection related activities.

If Vendor, do you require any site specific
training related to fuel movement?
Response - Site lifting and
rigging

When moving fuel in the SFP (non-outage)
what role / responsibility does Operations have at your site?
(On the bridge? – In the Area? – Remote
Monitoring? – Periodic Observation?- Other?)
Response - Operations is only
required to verify Tech Specs for fuel movement. All activities in the
fueling building would be oversight by Reactor Engineering, Refueling,
or Fuel Handling Supervisor.
2. What is the make-up
of the Fuel movement Crew? (Handler, Spotter, Coordinator, SRO?)
Response
- Site oversight, fuel handler, and spotter.
· What
groups participate in fuel movement and in what capacity?
Response
- Radiation Protection is in fuel building when fuel is being moved. There
may also be a dedicated FME Monitor supplied from maintenance.
· What
verifications are performing during fuel movement and who performs them?
Response
- Fuel handler, spotter and site oversight all verify verification of fuel
assembly moves.
· Do
you have site personnel qualified to move fuel or do you exclusively use
contractors?
Response
- Currently I do not believe we have any site personnel to move fuel. There
were some Operations personnel qualified, but I believe their qualifications
have expired.
3. Do you have a detailed
communication script for fuel movement?
Response - There is no specific
written details, only general three-way communication requirements. The
crews brief the specific communications at the start of the activity and
would adjust during each days activity if necessary.
4. What specific barriers
do you have in place to minimize the potential for a fuel mispositioning
event?
Response - Reactor Engineering
(RE) develops ALL fuel moves using ShuffleWorks. A second RE is required
to independently check the fuel moves, i.e. not using ShuffleWorks. Third
party verification is used in the field. We will also provide large SFP
printouts of the storage cells that includes the assembly IDs, locations,
rack indexing etc. This is mounted both on the SFP crane and on the wall
in the SFP for the verifiers. For our current SFP Re-rack Project that
is in-progress at BV2, we have also utilized the Westinghouse SureTrac
Fuel Pool Index System for the new racks. This is a laser system that mounts
on the crane and provides a digital readout to the crane operator.
5. What pieces of
information are included on your fuel movement sheets?
Response - Assembly ID, any fuel assembly insert information, SFP
location from and to, and any applicable notes from ShuffleWorks. ShuffleWorks
is used to track any special considerations for fuel movement and these
notes may be added to the printouts provided to the fuel movers. This would
include items like damaged grid straps, fuel failure, restrictions due
to fuel issue over the years - top nozzle separation, loose bottom nozzle
screws, damages guide tube flexure from a recon, etc.
· How
are verifications documented during fuel movement?
Response - Fuel Movement sheet
is initial and dated
6. Once the activity
is scheduled and movement sheets have been generated, what is the process
for moving a fuel assembly during a refueling outage?
Response - Movement sheets are
held by RE until they are needed. Operations verifies Tech Specs for fuel
movement. Pre-job brief. Fuel mover and Fuel Handling Supervisor utilize
move sheet and verify next fuel move. Spotter (usually up-ender operator)
provides a third party check. Position is given to latch and unlatch by
Fuel Handling Supervisor.
7. Once the activity
is scheduled and movement sheets have been generated, what is the process
for moving a fuel assembly during when not in a refueling outage?
Response - Similar to refueling,
except Fuel Handling Supervisor may be Reactor Engineering. The third party
verifier would additional spotter on the bridge crane.
