This is what we use for the Dominion Units (Surry, Kewanee, North Anna and Millstone):
3.9 Unplanned Reactivity Change Caused by Equipment Problem or Personnel Error
• Reactor Power Change >0.5% RTP When >5% RTP
• Reactivity Change >350 pcm When <5% RTP
4.6 Unplanned Reactivity Change Caused by Equipment Problem or Personnel Error
• Reactor Power Change < 0.5% RTP When >5% RTP
• Reactivity Change <350 pcm and >150 pcm When < 5% RTP

Here is what Duke Energy (Oconee Nuclear Station, Catawba Nuclear Station, & McGuire Nuclear Station) uses:
A positive change in reactivity GREATER THAN OR EQUAL TO:
· 0.05% ÄK/K (50 pcm) beyond intended change or expected tolerance (Mode 2 only) OR
· 0.35% ÄK/K (350 pcm) beyond intended change or expected tolerance (Mode 3, 4, 5, and 6
only)
A negative change in reactivity GREATER THAN OR EQUAL TO 0.15% ÄK/K (150 pcm)
beyond intended change or expected tolerance (Mode 2 only)
