1.         What is the purpose of your new fuel receipt activities? (indicate all that apply)

 

            a.         10CFR50 Appendix B.VII quality inspection? Yes, the receipt inspections are 10CFR50 Appendix B inspections.

            b.         Inspection for shipping damage and foreign material? Yes.  The receipt inspections focus on three areas of concern:  1. manufacturing defects, 2. shipping damage, and 3. debris.

            c.         Other (please specify):

 

2.         Which organizations at your plant perform the following functions relative to new fuel receipt?

 

            a.         Overall new fuel receipt project management. Fuel Performance Analysis group at Corporate

 

b.         Ensuring adequate resources from other groups (Security, Radiation Protection, etc.) are aligned for new fuel receipt. Fuel Handling group (comprised of Licensed and Non Licensed ROs)

 

c.         Ensuring appropriate Preventative Maintenance has been performed on new fuel receipt equipment (hoists, bridges, etc.).Fuel Handling group (comprised of Licensed and Non Licensed ROs)

 

d.         Providing supervision of craft personnel (crane and bridge operators, mechanics, etc.) involved in new fuel receipt. Supervisor, Fuel Handling group (comprised of Licensed and Non Licensed ROs)

d.         Performing actual new fuel receipt inspections. Currently 1 Fuel Performance Analysis person from our corporate location and one onsite RE.

e.         Interfacing with the fuel vendor.Fuel Performance Analysis group at Corporate

 

f.          Generating fuel movement control sheets or checklists.Fuel Performance Analysis group at Corporate

 

g.         Tracking Special Nuclear Material (including processing of Material Transaction Reports).Fuel Performance Analysis group at Corporate

h.         Verifying integrity of shipping vehicle and shipping container accelerometers.Radwaste Shipping verifys shipping vehicle integrity.   During receipt, one Fuel Performance Analysis person from our corporate location and one onsite RE.

Shipping container accelerometer status is checked by the fuel inspectors (i.e., Onsite RE/Fuel Performance Analysis personnel).
Shipping container seals are checked by security.

3.         What qualifications are required for individuals performing inspections of new fuel during new fuel receipt? (indicate all that apply)

            a.         Part of reactor engineering qualifications. Onsite, , we have Dominion Qualification Records specifically for reactor engineers. Fuel inspectors (Reactor Engineering and Fuel Performance Analysis-corporate personnel) are certified in accordance with Dominion's QA program which references the requirements of ASME NQA-1-1994.

            b.         Task-specific qualification card.  An inspector certification form is issued at the time of certification/re-certification.  Fuel inspectors are re-certified annually.  

            c.         ANSI N45.2.6-1978.  Dominion's QA program references the requirements of ASME NQA-1-1994.

            d.         Others (please specify): Fuel inspectors must pass an annual eye exam.

4.         What role does your site/fleet QA or QC organizations play during new fuel receipt?

             a.         Performs new fuel receipt inspections.
QA/QC does not participate directly with new fuel receipts.  

            b.         Qualifies new fuel receipt inspectors. We have QA/QC eye exams that we must pass to perform receipt inspections.
QA/QC does not certify/re-certify fuel inspectors.  Certification/re-certification of fuel inspectors is performed by the Supervisor of Fuel Performance Analysis (corporate).

            c.         Normal QA/QC oversight of activities.
QA/QC may perform an audit or surveillance on the receipt activities.  Supervision of receipt activities is provided by the Supervisor - OPS Fuel Handling group.

            d.         None.
